We are seeking to appoint motivated and knowledgeable Family Practitioners to join the 0-18 Help team. The Early Help Service supports families with children and young people aged 0 to 19 and up to 25 with a disability. The service works in partnership with families to address problems, or potential problems, at the earliest opportunity.  Early intervention may occur at any point in a child or young person’s life.

The successful candidates will be able work in partnership with other professionals, parents and carers to intervene early when families need more support. You will be expected to assess needs, complete an action plan and undertake Team Around the Family meetings (TAF). The successful candidate will also need to have excellent interpersonal skills to work with colleagues at all levels of the organisation and be competent at assessing risk and making sound casework decisions.
